Target Audience
This activity is intended for hematologists/oncologists, advanced practice providers, nurses, psychologists, social workers, and other healthcare professionals involved in the care of adolescent and young adult (AYA) survivors of cancer.
Description
This webinar, featuring a panel of subject matter experts, explores the unique challenges and opportunities to support adolescent and young adult (AYA) survivors of cancer. The role of primary care clinicians in managing long-term follow up care, strategies to strengthen communication between oncology and primary care teams, and resources to support AYA survivors, their caregivers, and healthcare professionals will be discussed, with opportunities for input from the audience.
Learning Objectives
After completing this activity, the participant should be better able to:
- Describe the unique survivorship challenges AYA patients face during care transitions and integration with primary care
- Explain the important role of primary care clinicians in delivering long-term follow up care for AYA cancer survivors
- Apply communication and care coordination strategies to strengthen the relationship between oncology and primary care teams
- Access tools and resources to support AYA cancer survivors, their caregivers, and healthcare professionals

As a Jointly Accredited Organization, Medical Learning Institute Inc (MLI) is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses are approved under this program. Regulatory boards have the final authority on courses accepted for continuing education credit. Social workers completing this course receive 1.0 Clinical Social Work continuing education credit.
