The Leukemia & Lymphoma Society's (LLS) Gateway Chapter Patient Travel Assistance program is available to blood cancer patients, with significant financial need, who may qualify to receive financial assistance for approved expenses which include: ground transportation (gas, tolls, car rental, taxi, bus, train, ambulance services, etc.), air travel, and lodging related expenses.
Support for this program is based on the availability of funds by disease diagnosis. Funds are available for the following:

Please note that patients can only receive travel assistance once per fiscal year (July 1 - June 30). If a patient received assistance through a national travel program, they will not be eligible to receive assistance locally.
**As a non-profit organization, we rely on the generosity of our sponsors. Program continuation is dependent on the availability of funds and the program could be modified or discontinued at any time if funding is limited or no longer available. Please visit our website in the future to become aware of new funding.**
Program Eligibility Criteria
- Applicants (patients) must be US citizens or permanent residents, and
- Applicants must have a confirmed blood cancer diagnosis. (Physicians signature confirming diagnosis is required), and
- Applicants must demonstrate financial need: household income at or below adjusted 500% U.S. Federal Poverty Level (FPL), (Financial documents such as recent tax returns, 1099s, Social Security, benefit statements or W-2s will be reviewed to determine eligibility).
If a patient has received travel assistance through a local or national travel program after 6/30/2017, they should not apply. Patients cannot receive travel assistance both locally and nationally.
