Target Audience: This activity is intended for oncologists and primary care providers (PCPs), such as internists, family and general practitioners, oncology nurses, pharmacists, physician assistants, and social workers.

Activity Purpose
The goal of this activity is to provide education about survivors of hematologic cancers and to facilitate shared care of these individuals by PCPs and hematologists/oncologists.
Educational Objectives
After completing this activity, the participant should be better able to:
- Use a survivorship care plan template to communicate multidirectionally with other providers and to coordinate care of cancer survivors
- Assess the special needs of a cancer survivor, in terms of prevention and screening, imaging, and laboratory tests
- Identify and manage long-term and late effects of treatment for hematologic cancers, including Hodgkin lymphoma and myeloma.
