The LLS Mission Advancement Award recognizes a health care professional or health care team that has:

• Provided outstanding support, counsel or guidance to those with a blood cancer and/or their families.
• Demonstrated outstanding dedication to promoting awareness and/or support related to blood cancers.

Eligibility

The following criteria would make an otherwise qualified candidate ineligible for this award:

• Receipt of this award in the past 3 years.
• Current employment by LLS or membership on any LLS Committee or Board.

Selection

Nominations can be made at any time by completing this form.  The Mission Advancement Committee will meet, review nominations, and select the recipient for this award.
